Bausch Health Exiting Medicaid Drug Rebate and 340B Programs

On October 1, 2025, Bausch Health voluntarily withdrew from both the 340B Drug Pricing Program and the Medicaid Drug Rebate Program (MDRP). This action applies to 12 labeler codes and hundreds of NDCs across Bausch Health’s portfolio, including Xifaxan, a high-cost treatment for hepatic encephalopathy and IBS, with no generic alternative.

Key Impacts

  • Patient Access – Patients with Medicaid will no longer have these medications covered. Bausch has stated it will continue a Patient Assistance Program for Medicaid patients who lose coverage, but eligibility will be limited (current program can be found here: https://www.bauschhealthpap.com/).
  • Loss of 340B Pricing – Covered entities will no longer be able to purchase affected Bausch drugs at 340B prices. Drugs such as Xifaxan could cost more than $3,000 per prescription at WAC.
  • Uncertainty on Scope – Some, but not all Bausch + Lomb eye-care NDCs are impacted, but hundreds of other Bausch Health and Salix NDCs are affected.

Action Steps for Covered Entities

  • Review Affected NDCs:
    • Labeler codes: 25010, 16781, 57782, 65649, 00095, 00187, 00884, 13548, 66490, 68012, 68682, 99207.
  • Coordinate with TPAs & Contract Pharmacies:
    • Proactively block terminated NDCs and prevent replenishment orders.
    • Cease submissions to ESP for the affected NDCs.
    • Since these affected NDCS are no longer Covered Outpatient Drugs, DSH, CAN, and PED entities may begin purchasing these NDCs on GPO accounts. Understand what changes your split-bill software provider might have made on your behalf, or talk to them about splitting these NDCs to your GPO account.
  • Identify Patient Impact
    • Review patients currently on Bausch Health drugs and assess alternative therapies or assistance program enrollment.
