The Georgia Department of Community Health (DCH) has posted the requirements to qualify for the Rural Hospital Tax Credit Program for calendar year 2017. This information is located at

A list of eligible organizations is included on the website, along with a financial proxy form and a form detailing the content of a five-year viability and stability plan. Each eligible organization is required to submit the completed forms to DCH no later than 5:00 P.M. on Monday, October 17, 2016.
