In recent days, HRSA sent a notification to many healthcare providers titled “Provider Action Required: Provider Relief Program Audit Reporting Requirement Attestation – Delinquent Audit Notice.”  This notification informs the recipient that they are required to complete an online attestation form by April 5, 2024.

The attestation form must be completed by an authorized representative of the organization and includes the following required information:

  • Organization information
  • Mailing address
  • Authorized representative information
  • Audit threshold reporting
  • Attestation of document review

Under the audit threshold reporting, authorized representatives must attest as to whether their organization met the $750,000 single audit threshold, and if so, must attest to which fiscal years the organization has filed the single audits for and which programs were covered by the single audits.  If the organization met the $750,000 threshold but is delinquent in reporting the single audit, the authorized representative must provide certain information related to the expected completion and submission.
